Part-Time Adult Life Planner

Trellis Services is hiring abd Adult Life Planner who will provide information and resources to support an individual accessing Autism Waiver Services and their family with transition planning as a means to successfully transition from In- Home Support Services/School Services into Adult Services.

• Identify client skills related to independence, self-advocacy, self-direction and employment options.
• Assist in connecting the family and client to support services and resources to help guide them into appropriate adult programs, volunteer opportunities and/or employment.
• Ensure the development of an Adult Life Planning Treatment Plan to guide services.
• Complete mandatory trainings and attend required meetings.
• Document services delivered accurately and timely according to company policies.
• Report clinical and scheduling concerns promptly.
• Provide clinical and professional feedback to staff.
• Adhere to all company compliance policies, written or unwritten.
• Complete other tasks as assigned by supervisor.

• Master's Degree
• 5 Years’ Experience in Special Education Services
• Awareness of various Adult Services - how to connect to services provided by the Developmental Disability Administration, Division of Rehabilitation Services, Department of Labor, Licensing and Regulation, Mental Health Administration/Behavioral Health Administration and Social Security Administration
• Awareness of local DDA funded adult service agencies
• Awareness of how to apply for and connect to disability support services at the post-secondary level
• Awareness of local community support groups
• Awareness of needed documents for long term financial support and decision making, and planning for adulthood including Maryland ABLE and Guardianship and the Alternatives to Guardianship
